East Middle School

The goal of this School Based Center (SBHC) is to help improve the health and well being of students and families. Healthy students are better equipped for school success.

The purpose of this program is to provide quality healthcare in a student-friendly setting, at a time that is convenient to the student and family. The Wellness Center is NOT trying to replace your regular source of healthcare.

To register, fill out forms and return them to your EMS office or to the Wellness Center.

By completing the forms, your child may be seen at the Wellness Center during the school day for medical concerns. NO child is seen at the Wellness Center without permission of the parent or guardian.

The Wellness Center will bill your insurance company for services your child receives in our center.

Parents will need to bring a copy of your child's health insurance card.
Services Available At No Cost Include:
Routine physical exams for sports/school Family and community health education
Immunizations (Vaccine for children Program Participants) Counseling and referrals to community resources
Diagnosis and management of acute illness & chronic diseases Individual, group, and family counseling
Dental, hearing, and vision screening Crisis intervention
